Making a referral

Who can we support? 

People diagnosed with an illness from which they are unlikely to recover. While the end of life may not be imminent, people can greatly benefit from hospice support, from the moment they receive their diagnosis.

Support is available for families, friends and colleagues to help them with the impact of a loved one's life-limiting illness. We are also able to support families following the death of someone cared for by North Devon Hospice.

What support is available?  

  • Emotional and spiritual support
  • Specialist, complex symptom and pain management
  • Personal care and support in people’s own homes
  • End of life care and support in your own home or on the hospice bedded unit
  • Bereavement support

When should the referral be made? 

The sooner the better. Early referrals to North Devon Hospice are vital, so support can be provided for the entire journey, from initial diagnosis and beyond.

Who can make a referral? 

Any health or social care professional who knows the patient or family member can make a referral. This could be a GP, district nurse, social worker, specialist nurse, consultant etc.
