Hospice to Home

When someone is nearing the end of their life, the person and their family members may benefit from having some extra support. Our Hospice to Home team visit people in their own home, offering support and guidance at a difficult time. During the coronavirus pandemic, the Hospice to Home team are still caring and have a caseload of people they are supporting.

The vast majority of people we meet tell us they would prefer to spend their final days at home and our Hospice to Home team are here to support those wishes where possible.
The Hospice to Home team offer end-of-life care and support for patients and their carers in their own home. The team travel the length and breadth of North Devon, visiting people at home, to provide round-the-clock care at the end of someone’s life. This can mean extra support for carers at such a difficult time; it can also mean that family and carers can have more opportunities to spend meaningful time with the person who is unwell.

Service description
Our team of Multi-Skilled Healthcare Assistants provide practical hands on end-of-life care and support in the community, caring and supporting patients in their own home or other place of care such as nursing homes and residential homes. Our skilled carers can provide round-the-clock care, by providing night sits and day visits, helping to prevent an unnecessary admission to hospital. The Multi-Skilled Health Care assistants are supported by a team of Clinical Nurse Specialists who are able to provide specialist support and work closely with the other health professionals involved in your care. This service supports people to spend their final days and hours at home, often surrounded by family and friends.

How to be referred
To be referred to any of our services please ask your GP or District Nurse for a referral.  For those patients who may benefit from having the Hospice to Home team visit them at home, it is likely that the healthcare providers involved in your care will discuss this with you and your family at the time.  It is your decision if you would like this type of support.

Service delivery
Our team of Multi-Skilled Healthcare Assistants work throughout the North Devon community, providing care and support in people's own homes and this includes visiting patients and their families in care homes.
