Home is where the heart is. If you are having difficulty managing symptoms like pain or sickness, or are in need of some hands-on care at home, then our community teams are here to support you wherever you call home.

Our Clinical Nurse Specialist team works alongside GP surgeries to support patients and families as well as professionals who are involved in your care.
Treating you with compassion and dignity, they will visit you at home to explore what your needs are, and how we can support you.

We can visit you at home, support you over the phone, or at either of our hospice sites (Deer Park in Barnstaple or The Long House in Holsworthy). How often your Clinical Nurse Specialist will visit you will be mutually agreed based on your needs at the time.
When you need more hands-on care, our Hospice to Home team of Staff Nurses and Senior Healthcare Assistants provide care and support for you and your family, or your main carers, in the privacy of your own home. We are here specifically for when you need more hands-on care and assistance, most commonly towards the end of life.

Practical advice and emotional support
How often our Hospice to Home team will visit is based on your care needs at the time. If your care needs suddenly change, we would aim to visit and support you as quickly as possible.
Our services are available to anyone in North Devon with a life-limiting illness, please ask your GP or hospital doctor for a referral.
