- The person being referred must give consent for the referral and for us to have the information we require to deliver our services
- We always recommend that you contact their GP before you send a referral
- Please provide the necessary information we require
Who is eligible for referral to North Devon Hospice?
Patients diagnosed with a life-limiting illness
This includes cancer and non-cancer diagnoses (such as COPD, heart failure, MND and others). Refer for:
- Symptom management relating to their life-limiting illness, such as pain, nausea, constipation, breathlessness and others
- Emotional, psychological and spiritual support due to the impact of their life-limiting illness
- End of life care
Families and carers
Friends, colleagues and relatives of a person with a life-limiting illness can be referred for:
- Emotional, psychological and spiritual support due to the impact of a loved one's life-limiting illness
- Bereavement support following the death of someone cared for by North Devon Hospice

Who can make a referral to North Devon Hospice?
Any health or social care professional who knows the patient or family member, such as GP, district nurse, social worker, specialist nurse, consultant.
We use SystmOne as our Electronic Record, for care providers also on SystmOne, we can accept Electronic c referrals.
